Extract the archive to the Witcher 3 main folder.
If you use NMM, you still have to manually copy modnewgameplusplus.xml to [Witcher 3 main folder]\bin\config\r4game\user_config_matrix\pc.
Rename [Witcher 3 main folder]\mods\modNewGamePlusPlus\content\en.w3strings to the in-game language if it isn't English.
